Anterior teeth trauma is one of the most common dental injuries. The type of injury and the condition of the injured tooth often dictates the treatment required. Reattachment of tooth fragment is one such treatment option that has a shorter treatment time and provides good aesthetics and function with relative ease and lesser complications in a less invasive way. Reattachment of tooth fragment can be considered one of the first treatment options over complete replacement procedures owing to its simplicity and conservation of tooth structure but the patient’s cooperation is of utmost importance for such a procedure. It is important to explain the limitations of the treatment and expected outcome. The present case describes the management of fractured mandibular anterior teeth by endodontic therapy and fragment reattachment. The reattachment of fractured fragment helped establish good esthetics in a relatively cost-effective and immediate way with good patient satisfaction. It is important for clinicians to be aware of such treatment modalities and use it in their day to day practice.
CASE: The root canal was dried with a paper point and sectional obturation was done up to 5mm from apex. The post space was prepared with Peeso Reamer, and a prefabricated
glass fiber post was luted in the canal with dual‑cure resin cement. The tooth fragment was disinfected and then rinsed
properly with water. To facilitate attachment of fragment to remaining tooth, a
slot was prepared in the tooth fragment so that it adequately fits on the coronal
portion of the fiber post. Acid etching of the enamel of
approximating surfaces of the two segments was done with 37% phosphoric acid
and rinsed. Then, resin cement was applied and both the
fragments were re-approximated and light cured for 40 s each from the buccal
and lingual aspects of the tooth. Finishing and polishing of
the tooth was done.
